I encourage all patients to contact their insurance company to determine the exact amount you will pay for out-of-network services prior to our visit.
Prior to our appointment, you will receive a link with access to your patient portal. This will have information about my office location, practice policies, privacy notice and intake forms.
Please complete these forms at least 48 hours prior to your initial evaluation or your appointment will need to be rescheduled.
At our first visit, I will ask you about your current concerns, clarify your mental health and medical history, review initial diagnostic impressions and discuss potential treatment options. Please bring a list of your prescription medications and any over-the-counter medications or supplements, as well as any important medical or mental health records. The first three visits are a consultative period to determine whether we are a good fit. If my expertise is not a good match for your needs, I can assist you with a referral to another provider.
My office is located in downtown Tacoma, so please arrive 10-15 minutes prior to your appointment to allow time for parking. There is metered parking on all surrounding streets, including behind the building in Court A, as well as on Pacific Avenue, Commerce Street, and S. 7th & 8th streets.
Patients must be vaccinated against COVID-19 to schedule an in-person appointment. Proof of vaccination will be required at the time of your first visit. Please wear a mask when entering the building regardless of vaccination status. No proof of COVID vaccination is necessary for virtual appointments.
All providers in our office suite are fully vaccinated against COVID-19. In addition, I run an air purifier in the room and sanitize high-touch surfaces between each appointment. At the beginning of each visit, I will ask about recent exposure to COVID-19, as well as any of the following symptoms: fever or chills, cough, shortness of breath or difficulty breathing, fatigue, muscle or body aches, headache, new loss of taste or smell, sore throat, congestion or runny nose, nausea or vomiting, diarrhea.
If you have recently been exposed to COVID-19, or if you have any of the above symptoms, please DO NOT come to your scheduled in-person appointment. Please call the office as soon as possible to discuss alternative options for your visit.

I currently offer both in-person and virtual appointments, if it is appropriate for your situation. I am happy to discuss options further during our screening phone call.
